ACCEPTANCE STATEMENT

By completing this subscription form, I understand and agree to the following terms.

SUBSCRIPTION DETAILS

• I am subscribing to the Unitus Therapy Intelligence (UnitusTI) cloud platform.
• My plan is annual, paid monthly (12-month commitment), and automatically renews for another 12-month term unless cancelled at least 30 days before the renewal date.
• At checkout only the one-time USD $100 setup fee is charged. Monthly billing for the base subscription begins after the first month of use and continues for 12 months.
• My base monthly charge is USD $50 for up to 5 users (this is the minimum monthly charge and will not fall below this amount).
• I may add additional users billed in packs of 5 users for USD $50. Example: up to 5 users = $50/mo; 6–10 = $100/mo; 11–15 = $150/mo; etc.
• The base subscription includes up to 100 active clients or students. Additional active clients are billed in packs of 100 for USD $50/mo. Example: up to 100 clients = included; 101–200 = +$50/mo; 201–300 = +$100/mo; etc.
• Additional usage is automatically billed based on account activity.

FEES & USAGE

• A one-time, non-refundable setup fee of $100 USD will be charged at the time of payment.
• Regular monthly billing will begin one month after registration and continue for twelve months.
• My payment method on file will be automatically charged each month for the base amount plus any additional usage fees.
• The amount selected at checkout represents my monthly minimum.
• Additional usage fees will be automatically charged and I will receive advance notice for review.
• Licenses are not sold individually and prepaid packs are non-refundable.
• Initial setup fees are one-time and non-refundable.

TAXES

Sales tax, VAT, GST, or similar taxes may not be displayed at checkout. Where required by applicable law, taxes may be calculated and charged on current or future invoices. Where such taxes are not collected by UnitusTI, the customer is solely responsible for reporting and remitting all applicable taxes.

DEFINITIONS & BILLING

• A "license" is counted for each "user" with active status at any time during the billing period.
• Users who are deactivated or archived for the entire billing period are excluded from the count.
• The term "user" refers to active individuals accessing the platform. Each user represents a therapist account.
• The term "active client" refers to each person (client, student, or beneficiary) with active status on the platform at any time during the billing period. Clients who are deactivated or archived for the entire period are excluded from the count.
• My payment method will be automatically charged each month for my base fee plus any calculated usage.
• Paid fees are non-refundable.
• Payments begin upon checkout. My personalized account will be activated within approximately 3 to 5 business days.

CANCELLATION

I may request to cancel the auto-renewal by submitting a support ticket. Cancellation requests take effect at the end of the current annual term if received at least 30 days before the renewal date. If the request is not submitted with that notice, cancellation takes effect at the end of the following annual term.
